These forms are completed by a certified medical professional that can make an appropriate diagnosis of a disability. To assist you and the doctor in completing this form, be sure to review the 
disability impact and recommendations for support sections first.
When the form has been completed, call 250-740-6446 to make a one-hour appointment with a Disability Services Education Counsellor to discuss your accommodation supports for school.
- Verification of Disability – This form is needed for students who do not have medical information to document their permanent disability. Your Dr. or medical specialist completes pages 6-8 only.
- Temporary Disability – Use this form if your disability is short term and not permanent.
- Learning Disability Guidelines – Use this form to get reimbursed for 75% of the cost of your psycho-educational assessment. You must be enrolled and be eligible for a student loan.
