Documentation
Services are available to students who have certified documentation of a permanent or temporary disability.
A temporary disability is a short term disability, injury or illness that has been verified by providing appropriate documentation from a physician or licensed professional. The documentation should indicate the nature of the disability, anticipated duration, and the services which would be most appropriate. Download the

A permanent disability means a functional limitation caused by a physical or mental impairment that restricts the ability of a person to perform the daily activities necessary to participate in studies at a post-secondary level or in the labour force and is expected to remain with the person for the person's expected natural life.
Depending on the disability, acceptable documentation should:
- Be dated within the past 5 years
- Outline the nature and extent of the disability
- Include a detailed explanation of the
disability impact that prevents the student from fully participating in post-secondary studies - Give explicit
recommendations for support and/or coping strategies to reduce the impact of the disability to a student in an educational setting.
Acceptable documentation can be one of the following:
- Verification of
Permanent Disability Form Dr. completes pages 6-8 only! - Opthalmologist Report
- Psychiatric Report
- Audiologist Report
- Psychological Report
- Psycho-Educational Report
Reimbursement of 75% of the cost of one diagnostic assessment for learning disabilities may be available to an enrolled student who can demonstrate at least one dollar of finanacial need.
When all necessary documentation is not available or psycho-educational assessment is in progress, one semester of short term accommodation may be provided.
