Research Forms & Templates

The Research and Scholarly Activity Office has collected the most common forms and templates that researchers may need. There are three types of forms and templates. The VIU Standard Forms and Templates for Research, the VIU Internal Research Funding Application Forms and Common External Agency Application Forms.

  • Institutional Signature Sheet - This form must accompany all external research grant applications, research contracts and the VIU Research Award funding applications. NOTE: VIU has an indirect cost guideline of 25% of cash contributions (i.e. over and above the full expenses of the grant). For more information please check the guideline here.
    Researchers are responsible for obtaining the Department Chair’s, Dean’s and Research Centre Director’s (if applicable) signatures on this form before submission to the Research & Scholarly Activity Office. A copy of the grant application or contract must be attached to this form when gathering signatures and when submitting to the Research & Scholarly Activity Office.
  • Indirect Costs Waiver Form - Use this form if you are requesting a reduction in the amount we charge outside organizations (other than NSERC, SSHRC and CIHR) for indirect costs on research grants and contracts. Guidelines for indirect costs recovery, distribution and waiver requests can be found here.
  • Budget Cash Flow Form -This Excel document has been created by Financial Services to assist grant holders with monitoring funds.
  • Research Ethics Application Form - VIU is committed to ensure that all research involving human participants, whether conducted on or off campus, is of the highest ethical standards. Accordingly, all research involving humans must first be approved by the VIU Research Ethics Board (VIU REB). Application for Ethical Review of any such research must be made to the VIU REB to ensure compliance with the Tri-Council Policy Statement on Ethical Conduct for Research Involving Humans (TCPS 2) and VIU Policy and Procedures.   • Animal Use Protocol Application Form - In all teaching, research and other activities, VIU is committed to the humane care and treatment of animals. Accordingly, all such activities that involve the use of animals must first be approved by the VIU Animal Care Committee (ACC). The ACC reviews Animal Use Protocol Applications for all teaching, research, and other activities with a view to minimizing the number of animals used, minimizing their pain and suffering, and ensuring that appropriate care is provided at all stages of the animal’s life. To this end, the ACC ensures that all such activities, whether on or off campus, comply with the current policies and guidelines of the Canadian Council on Animal Care/Conseil canadien de protection des animaux (CCAC-CCPA).   • Postdoctoral Draft Letter of Offer Template - Use this template if you are wishing to appoint a new Postdoctoral Fellow. Detailed information outlining the process for appointing a Postdoctoral Fellow can be found in procedure31.16 Postdoctoral Fellowships (under review).
  • Postdoctoral Draft Letter of Offer Extension Template - Use this template if you are wishing to extend a Postdoctoral Fellow. Postdoctoral Fellow appointments must be renewed on a yearly basis and cannot exceed 3 years. If you have any questions or concerns please contact the Director of Research Services.
  • Fellowship Draft Letter of Offer template - Use this template if you are offering a Fellowship Award. This is not a salary, but an award/stipend for an independent researcher. It could be for a grad student or a faculty member. The policy and procedure are being developed, in the meantime use the postdoctoral procedure to assist with the process.
  • Visiting Scholar Letter of Offer Template - Use this template if you are wishing to appoint a Visiting Scholar. The policy and procedure are being developed, in the meantime use the postdoctoral procedure to assist with the process.
  • Application form to be eligible to hold a research grant. Use this application form if you are not an employee of VIU such as an adjunct or emeritus faculty, Postdoctoral Fellow, Honorary Research Associate or, a non-faculty but a VIU staff member, and you wish to apply for a be the primary investigator on a research grant. You will also need to review the Guidelines to be eligible to hold a research grant.

Common External Agency Application Templates

Social Sciences and Humanities Research Council (SSHRC)

SSHRC requires several different documents to be attached and uploaded to their online application system. Below are some typical templates that provide a starting point for you; however, check the instructions for your particular funding program and adjust as needed. These templates have been formatted according to SSHRC guidelines (e.g. ¾ inch margins, 12pt Times New Roman Font, headers and footers). The text in blue are typical guidelines for the various sections but these need to be checked with specific instructions for your funding program.
